Thank you for choosing Coastline Orthopaedic Associates for your musculoskeletal and orthopaedic care needs. To better ensure that the new patient registration process is as efficient as possible, please prepare to bring the following items:

• Print and fill out all New Patient Forms
• Insurance Card
• List of your medications with dosages and any known allergies
• X-Rays, MRI’s, and test results from your Primary Care Physician/ Referring Physician
